A professional nursing service that provides expert nursing care in the community setting. The Community health staff will discuss and assess your needs with you, and help you to explore options available through: - Provision of direct clinical nursing care; - Comprehensive clinical assessments; - Teaching individuals and their carers how best to manage their daily care at home; - Provision of health information and education; - Monitoring of health status. Community nursing also provide assessments for eligibility for home care, meals on wheels and transport services. Advice and referral for specialist nursing care is also available, e.g continence and depression.
