The psychologist delivers psychology assessment and
management of patients with a particular focus on improving the
health outcomes and quality of life for those with complex chronic
conditions through integrated care planning, assistance with
patient's psychological factors impact on their condition and to
facilitate chronic condition self-management.
The program has a particular focus on supporting clients with
chronic conditions and preventing hospital re-admissions related
to chronic diseases.
The psychologist can assist a range of client groups with a
particular focus, but not limited to:
• working with individuals to make lifestyle changes to maximise
health and functional outcomes,
• addressing emotional and behavioural factors related to
adjustment to chronic conditions or injury,
• assisting individuals adhere to medical interventions and
treatment regimes
Referrals related to a mental illness should be directed to
mental health service providers
