Inclusion Criteria: Individuals aged between 18 and 65 (age requirements subject to review), who… …are not responding well to treatment for mental health illness by General Practitioners. … with severe and complex mental health issues. … with severe mental illness and has a significant levels of disturbance and psychosocial disability due to their illness that has a significant impact on their functioning. …with severe mental illness and significant functional difficulties that require coordination of care and support across multiple agencies, family, friends, support people, carers and providers. … with severe mental illness and would benefit from a multi-disciplinary team due to the intensive or complex nature of care required.
All individuals from the Stirling Catchment Area who have never been seen by mental health services are asked to contact or attend the Mirrabooka Community Mental Health facility.
All discharged clients who wish to engage with mental health services are asked to contact their previous case manager. If they no longer have the contact information for their previous case manager, individuals are asked to contact Osborne Community Mental Health.
Based on convenience for the consumer and their residential address, further assessment or treatment may be conducted at the Osborne Community Mental Health.
