A CDAMS assessment will include medical, allied health consultations and may require a home visit. The outcomes of these consultations and recommendations is then discussed with the client and if agreed to their family and G.P.

1.Expert clinical diagnosis
2.Information on appropriate treatments
3.Education, support and information
4. Direction in planning for the future
5. Information on dealing with day to day issues
6. Linkages for families and carers to other services or community supports
Self referrals, G.P or service provider referrals are all accepted, if self/service provider referred a letter is sent to the G.P to request medical information and results of recent investigations.
