The program provides the following evidence based services with the aim to build individual's ability to self manage their chronic condition:
Health Coaching - One on one counselling style session, non directive, assists with identifying priorities, barriers and enablers to change and develop goals and self management plans.
MY HEALTH, MY LIFE! Course. A four week group program. Involves information provision and discussions regarding symptom monitoring and management, relaxation, problem solving, goal setting and communicating with health professionals. Group sessions are complimented with 6 months 1:1 health coaching service. Ideal for building skills in self management.
Flinders Model of Chronic Care - One on One, structured interview style sessions using a number of tools to identify the consumer's problems and goals, and develop a care plan. Quit Education and Heart Foundation Walking groups are also available.
Referral Process: Referrals are accepted from any service or individual including General Practitioners, staff, social workers, family, friends or self referral The client must be aware of the referral and a willing and informed participant.
