Chronic Pain Clinic provides single-disciplinary, inter-disciplinary treatment, group programs and group skills programs – or a
combination of these modes – as coordinated on a case by case basis. The aim of this clinic is to reduce the risk of long term
disability through clinical assessment and where indicated, provision of treatment on a time-limited basis, for clients
presenting with moderate to severe levels of persistent pain. Integral to the service model is a focus on enabling clients to gain
an improved ability to self-manage their condition and the development of strong links with carers to support continuing
management in a primary care setting following discharge from a specialist service.
