Our nurses liaise with the client, their family and other health providers to develop a comprehensive care plan which will ensure that the individual's health and care needs are met. 

Our services are provided irrespective of age, race, religion, presenting illness or social and economic status. The only criteria we have are that a person has a genuine need for care and that their needs are able to be met by the services we can provide.

The District Nursing Gisborne provides domiciliary nursing care 7 days a week. City Of Melton

Visiting nurses are able to offer wound care, diabetic care, injections, oxygen therapy, collection of pathology specimens, catheterisation, administration of medications, monitoring of health status and some more complex personal care to clients in their own homes. Area covered includes Bacchus Marsh and Melton and surrounding areas.

A range of health education services are provided including:
Women's Health, Men's Health, Needle and Syringe program, Refugee Health and Sexual and Reproductive health education (i.e. School Sex Education program). Pap smears are available by appointment.

Bacchus Marsh Community Health Centre

The  Domiciliary Support service monitors the wellbeing of both mother and baby until you are contacted by the Maternal and Child Health Nurse service (usually within a week of discharge).


Following delivery of your baby, and discharge home, one of our Midwifery team will visit you at home to ensure the transition into home life with your new baby is as smooth as possible.


At least one home visit is provided to all new Mums. If you have gone home less than 24 hours after delivery, or require additional support, extra visits may be arranged.
