Occupational Therapy promotes a person's wellness and independence in his or her normal environment.

Referrals from self, carers, family members, health care workers, community agencies and GPs. 

Referrals from non-individuals require a SCTT Confidential Referral Cover Sheet, Summary & Referral Information and Consumer Consent to Share template.  GPs may submit a Victorian Statewide Referral Form. 

Referrals must include the description of issues including relevant medical history, presenting problems and other contributing histories and factors. The 'Risk' section should also be completed if relevant. 

Hume City Council and RDNS referrals must have a Care Assessment Record.

Failure to provide this information may result in a delay in processing the referral or the referral being declined.

The Child Health Team consists of: Speech Pathologists, Occupational Therapist, Psychologist, Audiologist, Paediatrician. Prioritity is given to: Children and families living in the Moreland Local Government area; those not receiving services or assistance from another early intervention service; Children aged 0-6years.
